1. Click “Program Management” on the left side menu
2. Scroll down on the sub-menu
3. Click the “Incident Reports” section on the sub-menu
4. Click the “+ New Incident Report” button to create a new incident report
5. Select an “Incident Type” on the dropdown menu
6. Set the “Date of Incident”
7. Choose staff membes involved in or responding to this incident, you can also search staff members
8. Set the “Time” of the incident
9. Type the “Location”
10. Choose a “Severity Level”
11. Set any “Immediate Actions Needed”
12. Use the checkmarls for “Medical attention required”, “Requires immediate follow-up” or “Safety hazard present” accordingly
13. Type the “Witness Information”
14. for example, the name and contact information of witnesses (if any)
15. Select all clients involved, you can search clients by name.
16. You can search clients by name
17. Provide a detailed description of what happened
18. Assign this incident to a member, just click “Select assignee…”
19. Set a “Resolution Due Date”
20. Set a “Related Program”
21. When finished, scroll down and click the “Submit Incident Report”
