Form Builder #
1. Click "Program Management" on the left side menu
2. Click "Forms" on the sub-menu
3. Locate the form and click the "Edit Fields" button
4. Click "Basic Identification"
5. Click "Click field types on the left to add them, or drag fields here to reorder."
6. Click "Edit field"
7. Click "Field Label"
8. Click "Field Type"
9. Click "Description"
10. Click "Required field"
11. Click "Required field"
12. Click "Map to Client Field"
13. Click "Emergency Contact Email"
14. Click "Click field types on the left to add them, or drag fields here to reorder."
